Care Navigation

For many people, the hardest part isn’t the health issue itself — it’s figuring out what to do next. Our CHWs sit down with each individual to understand their full situation, not just the immediate need that brought them to us.

First Conversation

A CHW reaches out to listen, ask questions, and understand the full picture — health, housing, finances, family situation.

Building a Plan

Together, we identify what matters most right now and create a simple, realistic plan — not an overwhelming checklist.

Taking the Next Step

Whether it’s scheduling an appointment, applying for benefits, or finding transportation — we help make the first step happen.

Community Resource Connection

Health doesn’t happen in isolation — it’s shaped by housing, food, transportation, and financial stability. Our CHWs maintain an up-to-date understanding of local resources across Georgia and connect individuals directly to what they need.

Healthcare Access

Primary care, specialists, insurance enrollment, and prescription assistance.A CHW reaches out to listen, ask questions, and understand the full picture — health, housing, finances, family situation.

Behavioral Health

Counseling, substance use support, and crisis resources.

Food & Housing

SNAP benefits, food banks, rental assistance, and emergency housing.

Transportation & Benefits

Medicaid transport, ride assistance, and help applying for public benefits.

Follow-Up & Documentation

Connecting someone to a resource is only half the work — the other half is making sure it actually helped. Our CHWs check back in, troubleshoot barriers, and keep referral partners informed every step of the way.

For Individuals & Families

Check-ins to see how things are going, help solving new problems as they come up, and a consistent point of contact who already knows your situation.

For Referral Partners

Updates on outreach attempts, services connected, and engagement status — so you know what happened after the referral, without extra work on your end.
