About Community Care Pathway
Community Care Pathway exists to make sure no one in Georgia has to figure out the healthcare and social service system alone. We connect people to care through trusted Community Health Workers — neighbors helping neighbors find their way.
Why We Exist
For too many individuals and families, getting help means juggling phone calls, paperwork, waitlists, and conflicting information — often while dealing with a health crisis or financial stress. A missed appointment, an unfilled prescription, or an unmet need for food or housing can quietly derail someone’s health.
Community Care Pathway was founded to close that gap. We pair people with trained Community Health Workers — people from their own communities who understand the systems, speak the language, and stay with them until they’re connected to what they need.
Our Approach
Community Health Workers (CHWs) are frontline healthcare navigators who share life experience, language, and culture with the people they serve. That shared trust is what makes the model work — CHWs aren’t outside experts giving instructions, they’re community members walking alongside someone through a process.
Our CHWs are trained to assess needs, build a clear pathway to the right services, and follow up consistently — giving referral partners visibility into what happens after a hand-off, and giving individuals a steady guide through a path that often feels confusing on their own.
Conduct in-depth needs assessments covering health, behavioral health, and social determinants.
Link individuals to local resources — healthcare, food, housing, transportation, benefits, and more.
Stay engaged through check-ins, ensuring services are accessed and barriers are addressed.
Every person we serve is treated with respect — never reduced to a diagnosis, a case number, or a statistic.
Our CHWs come from the communities they serve — bringing trust, cultural understanding, and lived experience to every interaction.
Care decisions are based on clinical need and social factors alone — never on race, ethnicity, or background.
Our Network
Community Care Pathway is built in alignment with Georgia’s broader community health ecosystem — including the Atlanta Regional Collaborative for Health Improvement (ARCHI), the Georgia Department of Public Health’s CHW initiatives, and statewide advocacy efforts working to strengthen access to care for every Georgian.
